We need some information to grant you a MyChart account. After you submit this request, it may take up to a week for processing and verification. When completed, you will receive an email or a letter with your activation code and instructions on how to activate your MyChart account. NOTE TO PARENTS - WA State Law requires that teens have their OWN portal account. Parents of patients age 12 through 17 years are asked to NOT sign up on behalf of their child. Instead teens will be offered a portal account when they are at a clinic visit. Parents may choose to request a “proxy account” which allows you to access your child’s portal after they have set up their own portal account.
